Use this form to submit companies or installers to add to CableSux's Satellite Company Rating System.

Please provide as much information as possible. Do not submit any review information; you will have the chance to do that once the company/installer has been added to the database.

PLEASE use proper capitalization and formatting. It saves me a lot of time not having to make corrections. Phone #'s should use 999-999-9999 format; states = 2 letters (I.E. MI)
